• The fillable workbook (PDF): 15 pages in US Letter and A4, with 48 answer fields. Type into it in Adobe Acrobat Reader, or print it and write by hand.
• 10 guided sections: a name and the details only you notice, the people who share the responsibility, the day as it actually happens, comfort, current instructions, a folder someone else can find, appointments with a named next step, following up, a handover that can be read back, and keeping the plan current.

What it isn't
